Composite Dental Risk Score Across 5 Dimensions
One quarterly score per patient thattells you who to call first.
The DS-4 Risk Engine computes a composite 0;100+ dental risk score from five weighted dimensions: periodontal disease stage, diabetes control, high-risk medications, behavioral factors, and access/utilization. Scores are re-computed quarterly with prior-score deltas, trend lines (improving, stable, worsening), and a priority outreach flag that fires when a score climbs 10 or more points.
Periodontal disease, diabetes, high-risk meds, behavioral, and access factors each contribute points; the total is a transparent, auditable score.
Each score is paired with the prior quarter's value, the delta, and an improving/stable/worsening trend line so progress is visible.
Any score that climbs 10 or more points in a quarter auto-flags the patient for priority outreach by the assigned care manager.
Projected annual dental cost and projected medical cost impact are computed from the score, surfacing ROI of intervention.
Periodontal charting, restorative history, oral pathology, radiographic findings, and visit history are ingested from the dental record intake.
Active diagnoses (diabetes, CVD), medication list, and lab values are pulled from the patient record and matched to the dental findings.
Engine computes the 5-dimension composite, assigns a risk tier (low/moderate/high/critical), and flags priority outreach where needed.
Score, trend, recommended interventions, and projected cost impact are surfaced on the dental population dashboard and assigned care manager's queue.
“We picked up four high-risk seniors whose risk score jumped 18 points in one quarter. Two of them had undiagnosed periodontal disease driving medical spend.”
